Dennis O'Neil, 13, was beaten to death by his foster father, Reginald Gough, at Bank Farm,
Shropshire. A post-mortem examination revealed he had been starved for months and weighed just four
stone. The murder trial revealed that he had sucked the farm cows' udders in a desperate attempt to
get some sustenance. The case shook a war weary Britain and there was a national outcry when Gough
was jailed for six years for manslaughter. An appeal court ruling changed the verdict to murder and
his sentence was extended to 10 years. A Home Office inquiry identified a string of failures by the
staff and agencies involved in the case. There had been confusion between the two local authorities
responsible for the boy's foster placement, conflicting reports by childcare staff about his
wellbeing, staff shortages and miscommunication.

1973

Maria Colwell, seven, died in Brighton after being starved and beaten by her stepfather, William
Kepple. She had suffered brain damage, a fractured rib, black eyes, extensive external bruising and
internal injuries. Maria had been fostered by her aunt and uncle because her mother, Pauline, could
not cope with bringing up five children on her own. Five years later Pauline decided she wanted her
daughter back. But an inquiry by the Department of Health found that East Sussex county council had
insufficient evidence to return the girl. There were 50 official visits to the family, including
from social workers, health visitors, police and housing officers. All agencies involved in the case
were criticised.

Jasmine Beckford was starved and battered to death by her stepfather, Maurice Beckford. He was found
guilty of the four-year-old's manslaughter and jailed for 10 years. Her mother, Beverley Lorrington,
was jailed for 18 months for neglect. Jasmine had been in the care of Brent social services for
two-and-a-half years before she died, after Beckford was convicted of assaulting her younger sister.
She was seen by a social worker only once in 10 months.

Tyra Henry died after being battered and bitten by her father, Andrew Neil, while in local authority
care. Neil was subsequently sentenced to life imprisonment for the 21-month-old baby's murder. A
report on the case found that the white social workers from Lambeth council tended to be too
trusting of the family because they were black. John Patten, then a junior social services minister,
published new guidelines on child abuse cases for social workers soon after.

Heidi Koseda starved to death in a locked room in Hillingdon, west London. Her stepfather, Nicholas

manslaughter and detained in a high security psychiatric hospital. A private inquiry into her death
found that the senior National Society for the Prevention of Cruelty to Children inspector allocated
to her case failed to investigate a complaint of child abuse made by a neighbour. He also tried to
cover this up with a fictitious account of a visit to see the child.

But an inquiry found that her death was avoidable and concluded that four key social work and health
staff in Greenwich failed to apply the necessary skill, judgement and care in her case.

Doreen Mason (aka Aysha Kuddissi my angel) died of neglect after her mother and her boyfriend
bruised, burnt and broke the 16-month-old's leg then failed to have her injuries treated. Christine
Mason and Roy Aston were convicted of manslaughter and cruelty and each jailed for 12 years. Doreen
was on the "at risk" register of Southwark council . She slept on the floor where the couple put
junk food for her to eat. A report said her social worker was inexperienced and given no proper
training or supervision, and that Southwark social services department suffered from a "siege
mentality" and "destructive mistrust" between senior managers.

Leanne White, three, was beaten to death by her stepfather, Colin Sleate, who made her sleep on the

the stomach. Sleate was jailed for life for the girl's murder while her mother, Tina, received 10
years for manslaughter. An inquiry concluded that her death could have been prevented if
Nottinghamshire social services had responded properly to reports from her grandmother and
neighbours that she was at risk.

Rikki Neave, six, was found strangled by his coat zipper in a wood near Peterborough. His drug
addict mother, Ruth, was jailed after admitting cruelty towards Rikki and two of his three sisters.
She hit them, burned them, threw them across the room and locked them outside. Neave had asked a
succession of social workers to take the boy off her hands and told one she would kill Rikki if they
did not do something. A report by the social services inspectorate three years later said fault
primarily lay with senior management in Cambridgeshire social services department.

Chelsea Brown, two, was battered to death by her father. Robert Brown, who was jailed for life for
her murder, had a criminal record for violence against children. Her mother, Maria Brown, was jailed
for 18 months for child cruelty. The girl's social worker, Norma McDevitt, visited the family 27
times in the 10 weeks before her death. She took Chelsea to a paediatrician who said that six out of
nine areas of bruising "had no plausible explanation" and at least one was deliberately inflicted.
These findings should have triggered police involvement and a multi-agency case conference under
Derbyshire county council's procedures, but neither happened.

suffering months of horrific abuse and neglect. Her aunt, Marie Thérèse Kouao, and her boyfriend,
Carl Manning, were both jailed for life for the girl's murder in January 2001. A public inquiry into
her death began in September 2001, which is expected to lead to sweeping reform of Britain's child
protection services. It has heard that there were at least 12 chances for the agencies involved in
her protection to have saved her. Two social workers from the London borough of Haringey have been
suspended and face disciplinary proceedings.

services department has admitted it made serious mistakes and missed chances to save Lauren. An
inquiry found that inter-agency coordination was "ineffective" and social workers had not acted with
"due urgency".

Dennis Henry. The couple, from Plaidstow, east London, were jailed for manslaughter for deliberately
punching, scalding and burning the toddler, who had 64 scars and bruises on her body when she died.
She weighed just 9.5kg (21lbs), about half the normal weight of a child that age. An inquiry into
her death found that the health and social workers who should have protected her failed to do so
because they were paralysed with fear of Leanne and Dennis. It criticised the staff and agencies
involved for poor communication and for failing to carry out a proper assessment of the risks facing
Ainlee.

Summer Marie Phelps died at the hands of the people that were suppose to care for her the most, her
dad & step mom. On March 10th 2007 Summer passed away she was only 4 yrs old. She endured months of
toture by abuse. She was made to wash clothing in a bathtub with her feet against the tub & if she
moved from that position she was hit with a mop handle! They used a shock collar on her, she was
denied food, she was bitten, & beaten!

Chloe Thomas was 14 weeks old when she died in 2003 with 40 broken bones.

A coroner returned an unlawful killing verdict on Friday. Chloe's parents, from Bridgend, had both

A 30-year-old unemployed man received two life sentences today for murdering his girlfriend and her
six-year-old daughter.

Mark Nicholas, 30, admitted stabbing to death 33-year-old Nicole Batten and suffocating her daughter
Ukleigha Batten-Froggatt. The bodies were discovered at their home in a flat in Ossulston Street,
Camden, north London, after police forced their way in. Concerns had been raised by social services
because Ukleigha, who was on an at-risk register, had not been seen at school for a week.

The couple were found guilty at Lewes Crown Court, East Sussex on Monday.

It has now been confirmed that social workers in neighbouring Bedfordshire were tipped off that
Halling was abusing the
little mixed-race boy some six months before the murder.

Mistake number one came when they failed to pick up the signs of Halling's racist attacks, which
included stubbing out cigarettes on the little boy's body and calling him racist names.

Instead they believed the cover-up stories from terrified mum Donna, even when, according to
sources, she presented herself "black and blue" with bruises from Halling's attacks on her. Three
months before Deraye's death, Donna finally admitted Halling was beating her and fled to safety in
Milton Keynes.

An independent inquiry has concluded that the death of a three-year-old girl "could have been
prevented". But it refused to apportion blame to health and social work officers saying that "her
violent death could not have been accurately predicted".

Kennedy McFarlane died on 17 May last year after a blow from her mother's boyfriend sent her
crashing into the leg of a bed. Thomas Duncan, 33, was sentenced to life for the child's murder at
the High Court in Edinburgh in November.

The independent inquiry, which was carried out by consultant paediatrician Dr Helen Hammond, said
the child's life could have been saved if Dumfries and Galloway Council implemented its full child
protection procedures.

But it refused to blame any one individual or agency for her death.

Plymouth avoids child death inquiry

Plymouth Council has won a High Court battle to prevent an inquiry into its role in the death of a
baby whose mother was jailed for cruelty.
A judge quashed a coroner's call for an investigation into the role of "systems neglect" in the
death of nine-month-old Perrin Barlow.

Mr Justice Wilson said that, while taking Perrin into care would have saved him, the council and
other agencies had no reason to suspect there was a "real and immediate" threat to his life.

Perrin died from bronchopneumonia, which may have been triggered by dehydration and malnutrition,
while on the child protection register and under an interim supervision order.

His mother, Stephanie Horrocks, and her partner, Mark McAndrew, were jailed last February after
pleading guilty to child cruelty. Horrocks later died in custody.

South Devon Coroner Nigel Meadows had said the inquest should cover the possible role of child
protection agencies, citing his right to life under the European Convention of Human Rights.

But Mr Justice Wilson said such an investigation could only be triggered if agencies had tangible
grounds for believing Perrin was in immediate danger.

He said that, apart from Perrin's low weight, there was "no history which gave rise to significant
alarm about Perrin's health or safety, let alone in terms of a threat to his life".

The stepfather of a three-year-old girl has been jailed for eight years after shaking her to death.

Stuart Pirie, 32, of Ripon Close, Scunthorpe, pleaded guilty to the manslaughter of Jasmine Galyer
after abusing her over a 13-month period.
Hull Crown Court heard how Pirie shook Jasmine after she wet the bed, causing brain injuries
equivalent to a child being involved in a car crash.
Mother Melissa Galyer, 21, was jailed for two-and-a-half years for cruelty.
The court heard that Jasmine was taken to Scunthorpe General Hospital on 5 February.
Pirie had told paramedics that Jasmine had fallen in the bathroom, though doctors immediately
recognised she had suffered critical injuries.
The youngster died during surgery after being transferred to Sheffield Children's Hospital.
Doctors found 51 bruises on her body more than 24 hours old and eight recently inflicted.

Social workers have been criticised for failing to prevent the death of a two-year-old boy who drank
his heroin addict mother's substitute drug.

Ashley Chadburn sipped from a bottle of methadone as Rachel Hipkiss smoked heroin in her home in
December 2001. When he fell ill, she left him in the care of a 16-year-old addict and went Christmas
shopping.

Hipkiss, 23, was jailed for six years after admitting to manslaughter at Sheffield Crown Court last
year. An independent report into the case has accused social services of being "too tolerant of such
serious drugs misuse".

In his report Roger Thompson, a former director of the NSPCC, makes 26 recommendations aimed at
preventing a repeat. "There was not, in my view, a full and proper assessment of the mother's
parenting capacity," he said.

The chairman of the Area Child Protection Committee, which commissioned the report, said that the
committee had already implemented half of the recommendations.
An inquiry into the death of a two-year-old boy who swallowed a bottle of his mother's methadone
found a "tolerance of drug misuse" among social services staff.

But the independent report concluded the tragic death of Ashley Chadburn, from Frechville,
Sheffield, could not have been avoided.

Five social workers involved in the case of an 11-week-old baby who was killed by his father learnt
yesterday that they will not face disciplinary action.

Caleb Ness from Edinburgh was known to be at risk when his father, Alexander, killed him by shaking
him violently.

After his birth Caleb was released from hospital into the care of his mother, who was a drug addict,
and his father by Edinburgh City Council. He was the subject of two child protection orders when he
died on October 11, 2001.

Chloe Fahey, the five-year-old girl found stabbed to death in her home in Stretford.

As social services bosses began an investigation, her distraught father spoke of the joy his
daughter had brought her family in her short life.

David Fahey, 31, said: "Chloe was a wonderful, happy, carefree child. She will always be in our
hearts."

Residents saw Mr Fahey sobbing uncontrollably outside his home following the discovery of Chloe's
blood-soaked body in the bedroom of an end-terrace home in Bradshaw Lane at 4am yesterday (Friday).

Officers arrested her mother at the scene.

It was the second police visit to the home in 24 hours, the Manchester Evening News understands.

The previous morning, officers arrested Chloe's mother for an alleged breach of the peace and she
was taken to the Manchester Royal Infirmary with minor injuries. Police then handed her case to the
"appropriate authorities" - believed to be social services.

It is understood Chloe's two-year-old baby sister was staying with her family at another address
when the incident happened.

After the discovery of Chloe's body, officers also arrested her father and another man outside the
property but they were released without charge within hours.

Det Supt Simon Barraclough said: "Our investigation is centred around establishing exactly what
happened in the house that night and finding out how Chloe died.

"However, further independent investigations will be carried out into all associated agencies to
establish the chain of events at Bradshaw Lane in the previous 36 hours.

"We also called at the house in the early hours of Thursday morning and arrested a 23-year-old woman
to prevent a breach of the peace. We then took her to the Manchester Royal Infirmary for treatment
to a minor injury to her hand.

A SOCIAL worker who failed to halt the abuse and murder of eight-year-old Victoria Climbie won back
the right to work with children yesterday. Lisa Arthurworrey, who repeatedly failed to notice that
Victoria was being tortured, was said by a tribunal to be 'another victim' in the Climbie tragedy.

The decision to overturn her ban on working in childcare means that every social worker, manager and
police officer responsible for failures in the Climbie case is now free to work again protecting
children. Miss Arthurworrey said yesterday that she felt vindicated by the ruling of the Care
Standards Tribunal, which reversed a ban imposed by former education secretary Charles Clarke

The tribunal panel blamed not the social worker but leaders and managers at Haringey Council in
north London for their 'chaotic' supervision.


Victoria, who died in February 2000, had been sent from the Ivory Coast by her parents to live with
'aunt' Marie Therese Kouao in the hope of a better education. Her body carried 128 injuries, and she
spent her final weeks lying in a bath, freezing and bound head and foot inside a bin bag. Kouao and
her boyfriend Carl Manning are serving life for her murder.

Courtney Crockett was beaten black and blue by Gareth Rees during a sustained bout of violence after
he moved in with her mother last Christmas.

Courtney’s natural father Paul phoned social services days before she died after noticing
extensive bruising on the child’s face and bottom.

But the social worker was on holiday and never returned his call – and Courtney died the following
week.

Carla-Nicole Bone 13 months old

THE family of murdered infant Carla-Nicole Bone last night condemned as a "whitewash" a report
clearing care workers of any blame for her death.

The grandparents of the 13-month-old girl said they were "appalled" by the findings of an
independent investigation into her death at the hands of her mother’s boyfriend.

Although the report said that care workers should learn lessons from the case, it concluded they
could not have predicted, nor prevented, Carla-Nicole’s murder.

Carla-Nicole died after a frenzied attack by Alexander McClure, 27, a labourer, at a remote cottage
which he shared with the child’s mother near Huntly, Aberdeenshire, in May last year.

He picked up the child by the legs and repeatedly "swung her like a pendulum" against a wall,
crushing her skull.

McClure was jailed for life last September. Carla-Nicole’s mother, Andrea, 20, was jailed for
three years for failing to protect her daughter from him.

The death of Chelsea Brown has again called into question the intervention of social services in
cases where children are known to be at risk.

'Evil' father Robert Brown was given a life sentence on Tuesday for what police have described as
one of the "worst cases of child abuse they have ever seen".

Mental disorder: Ntsimbi Galtricia is in hospital with knife wounds
A mother is suspected of stabbing her daughter to death four days after being cleared to keep the
child by a mental health team.
The girl, two-year-old Balthous, was found dead just after midnight by paramedics who were called to
her flat in Barking.
The mother, named locally as Ntsimbi Galtricia, 23, was suffering from serious stab wounds and
detectives are waiting to question her in hospital. Police say they are not looking for anyone else.

Social workers assessed Ms Galtricia last Thursday but allowed her to keep the child. They stepped
in after residents voiced concerns about the woman's strange behaviour. She was arrested on
Wednesday for harassment of local shopkeepers and police visited her again following another
complaint on Saturday.
Law House, the block of flats where French- speaking Ms Galtricia, believed to be from West Africa,
had lived with her daughter for around eight months, was cordoned off today. She was well known by
shopkeepers close to the nine-storey block, particularly those at the Thames Superstore.
Manager Huseyin Alacayir, 26, said he had called police to remove her a number of times. She was
eventually banned from entering the premises after failing to heed their warnings.

A London council has been criticised for "significant shortcomings" over the death of a 13-year-old
girl who was working as a prostitute when she was in its care.
Aliyah Ismail was found dead in her boyfriend's rundown basement flat in London on 18 October last
year after taking an overdose of the heroin substitute, methadone.
just a few days earlier the authorities had lost track of her after she ran away from a children's
home.

Failure to listen

An independent report into the case by childcare expert Maddie Blackburn found childcare bodies
responsible for Aliyah did not provide "a sufficiently effective response".
In particular, the report, published on Monday by the Harrow Area Child Protection Committee,
highlights the agencies' failure to listen to Aliyah.

She is reported to have suggested as early as March that she was involved in prostitution and
alleged that she had been abused by friends and relatives, although she later withdrew some of these
allegations.

The report says: "More attention appeared to be given to the needs of the adult family members
rather than to Aliyah."

The report also raises "fundamental questions" about how social workers should deal with cases of
children who fall into prostitution and drug addiction. The council has been criticised for failing
to impose a secure accommodation order on Aliyah which was obtained in the month prior to her death,
but not acted upon until 16 October when she had disappeared.

The council accepts criticism that it failed to listen to Aliyah and did not follow its "fast-track"
procedure to get her into secure accommodation.

The details of Letitia Wright's last agonised weeks of her life, before she finally died, were
revealed to Bradford Crown Court where her mother, Sharon Wright, 23, and her boyfriend Peter
Seaton, 22, are standing trial accused of her murder.
Leticia Wright was covered in bruises from head to toe, and had been burned by cigarettes, which had
been held against her young flesh for at least ten seconds each time - her hair had been torn out in
clumps - she had bruises on top of bruises - her scalp was covered in oozing, infected sores - she
had swelling and bleeding to her bowel and pancreas which would have caused her pain until her death
- she had been "branded" by a lighter - her blood was found throughout the house, even on a pair of
"furry handcuffs" - her left eye was blackened and closed - and she finally died from kicks and
punches to her stomach and skull which turned her head into a "boggy mass"!

Sumairia Parveen, 24, burnt 17-month-old Tahla Ikram with a cigarette and sliced open his leg
repeatedly until his tendons and veins were exposed.
Today (Friday, September 7), at Southwark Crown Court she was sentenced to nine years in prison for
causing or allowing the child's death.
Tahla's father, Abid Ikram, 30, was also sentenced to nine years for the same offence.
During sentencing, the judge said both must serve at least half before they are eligible for
parole.

Baby death pair 'will be jailed'
Parents of a 10-month-old boy who died from massive internal injuries on Christmas Eve have been
warned that a jail term is "inevitable".


Despite being on the "at risk" register, social workers failed to detect any bruising or injuries on
a visit two months before Neo's death because he was covered in baby powder.

A post-mortem examination also showed that as well as his fatal injury he had suffered rib fractures

The mother of a four year old boy who was stabbed to death during a weekend visit to his father's
home has been left "absolutely devastated" by his killing, friends have said.
The frenzied attack on Ryan Hawkins came a week to the day after he celebrated his fourth birthday.
His sister Donna, 14, who had called to collect him, survived the attack despite having more than 20
wounds inflicted.

Ryan was stabbed to death at a house in Royd Street, Hill Top, Slaithwaite, on September 23.
Donna survived despite suffering 23 stab wounds.
Their father, Christopher Hawkins, 47, has been charged with murder and attempted murder and is in
custody awaiting trial next year.

Police say Courtney Simpson stood by and watched as her boyfriend beat her four-year-old son to
death inside an east side motel room Friday.
Montgomery was arrested on murder charges and Simpson was arrested on A felony neglect of a
dependent charges after the death of Simpson's four-year-old son Elijah. The prosecutor's office has
filed a 72-hour motion. No formal charges have been filed. Prosecutors are meeting with the
detectives assigned to the case and reviewing evidence.
Doctors say Elijah Simpson was already dead upon arrival from a skull fracture Friday. Metro
homicide detectives found restraint marks on Elijah's wrists and ankles. Detective Markus Kennedy
says Simpson eventually admitted her boyfriend hog-tied Elijah for sucking his thumb.
Simpson now says she's sorry and says her son wasn't the only victim.
Indianapolis Metro Police say Simpson watched Montgomery pick up and threw Elijah to floor. The
little boy suffered a fractured skull, went into seizures and later died.

An expert has asked why a teenager who died of a heroin overdose was permitted to live with her
drug-abusing family.

The mother and half-sister of Carly Townsend, 16, were convicted of manslaughter through gross
negligence after failing to call help for her.

Martin Blakebrough, chief executive of drugs charity Kaleidoscope, questioned the role of social
services in the case of the Llanelli family.

Swansea Local Safeguarding Children's Board said it was conducting a review.

A Swansea Crown Court jury found both Andrea Townsend, 46, and Gemma Evans, 25, guilty after hearing
that they knew Carly had taken heroin, but put her to bed instead of calling an ambulance, thinking
she would "sleep it off".

Evans helped her half-sister buy the drugs which killed her and Carly had been hospitalised twice
for heroin overdoses.

The jury also heard how Carly's father had died through drug misuse alongside about 20 of her
family's friends.

The tragic death of toddler Trae-Bleu Layne who died of methadone poisoning has forced an overhaul
of practices by the council’s child protection service.
The department came under fire following the double tragedy in which the three-year-old died of
methadone poisoning in October 2006 and her mother Donna Gill, of Rembrandt Way, Coley Park, died of
an overdose just seven months later.
But representatives from Reading Borough Council’s children’s services department reported on
the improvements they have made since the tragedy at the Education and Children’s Services
Scrutiny Panel on Thursday.
Before the incident there had been 84 attempted visits to Miss Gill as Trae-Bleu was under the
council’s child protection plan (CPP), but only 25 of the visits were successful.

Judge David Hodson said Alisha Allen's death might have been avoided if her parents had received the
proper help.

He spoke out after hearing how she was removed from the at risk register by social workers only
weeks before she was shaken 'like a rag doll' by her father Gary Allen, 26.

Alisha was classified as a ' vulnerable' child even before she was born in August 2006.
But when her parents moved home a month later their new local authority decided instead to treat her
as 'a child in need'.

Although they were still visited by social workers and a health visitor, Alisha's progress would
have been monitored more closely had she remained on the at risk register.

In January 2007, Allen picked the crying toddler out of her cot and in a fit of rage shook her with
such force that she never recovered from her brain injuries.


She died three days later in hospital.

Joanne Hill, 32, drowned Naomi, who had mild cerebral palsy, in a bath but denied murder on the
grounds of diminished responsibility.
During a two-week trial at Chester Crown Court a jury heard that hard-drinking Hill was ashamed of
Naomi's condition, which meant the little girl had to use calipers to help her walk.
Judge Elgan Edwards sentenced Hill to life in prison - to serve a minimum term of 15 years.
Mr Edwards told her there was no excuse for what she did.
"You killed your own daughter because you could not cope with her disability," he said.
"You had other pressures upon you, a disintegrating marriage and you decided to kill your own
daughter by drowning her.
"This has been a very sad case. Sad for you, for your husband, for the child you killed."
Hill was taken straight to prison, rather than returned to a secure unit, because of her murder
conviction.
But the judge commented on her "sad" history of mental health problems and said he hoped she would
be transferred back to hospital very soon.

An educational social worker went to the home of seven-year-old Khyra Ishaq and her five brothers
and sisters in Handsworth, Birmingham, shortly after they were removed from school but got no
reply.
Khalid Mahmood, the MP for the area, claims that no follow-up call was made over the next two months
and no one went round again.
Yesterday he demanded an inquiry into the alleged failings in the lead-up to Khyra being taken to
hospital in a severely emaciated state.
Khyra, her two sisters and three brothers, were discovered at their two-bedroom terrace in the early
hours of Saturday morning.
Ambulance crews tried to resuscitate her but it is believed she was already dead when they arrived.
The other children, aged between four and 12, were lying on mattresses on the floor next to their
sister and were taken to hospital. Birmingham City Council said yesterday that they had been placed
in foster care.
The children's mother, Angela Gordon, 33, and her boyfriend, Junaid Abuhamza, have been charged with
neglect and remanded in custody.

Arrests after toddler dies and his brothers are hospitalised after taking parents' methadone

Two men have been arrested after a two-year-old boy died after overdosing on his parents' methadone.


West Midlands Police said the men had been freed on bail pending further inquiries into the death of
Brandon Davis, who died at Birmingham children's hospital, last Thursday.

Brandon's two brothers, who also swallowed the heroin substitute, were in hospital yesterday – one
is seriously ill.

Detectives have refused to specify whether the arrested men are related to the children.

A 30-year-old man was arrested on suspicion of neglect and a 33-year-old on suspicion of possession
of a controlled drug.

It is thought the children took the drug after it was mistaken for cough mixture.

A drug addicted couple have been jailed for five years each for causing the death of their baby
son.
The Old Bailey heard ten-month-old Neo Craig died from a punch to the stomach and had been dead for
hours before an ambulance was called last Christmas Eve. His blood and hair roots showed a cocaine
contamination from birth.
Judge Richard Hone, QC, said they claimed they loved Neo "but the bitter truth is that you loved
crack cocaine more."
Judge Richard Hone, QC, said Neo was on the local authority child protection register at Havering,
East London, following "unexplained" scratches and bruises.
The judge listed the catalogue of severe injuries Neo sustained later, including fractures to ribs
and his right thigh.
His parents were spending £250 a day on their crack cocaine habit, funded by a £60,000 inheritance
which Neo's mother, qualified nurse Sharma Dookhooha, received.
Scrap dealer James Craig, 26, and Dookhooha, 25, admitted causing or allowing the death of their
son. The prosecution dropped a murder charge.

Social services have admitted to failings in their care of a baby who was killed by his father in
south west Scotland.
Dumfries and Galloway Council said a mistake was made in not placing Dylan Lockerbie on the child
protection register.
Ian Metcalfe was found guilty on Tuesday of the culpable homicide of Dylan, who died when he was
five months old in March 1996.
Metcalfe, 34, was also convicted of assaulting and killing another son, Kyle, aged 10 weeks, in
1988.
Social services were involved with the family after Dylan's birth but did not consider him
specifically "at risk", it has emerged.
Since then, a review of the case judged that a mistake was caused by weaknesses in service
arrangements and an error of judgement.
However, the review did not conclude that Dylan's death would necessarily have been avoidable.

A review has begun into the care given to a four-month-old baby, who was killed in a fire started by
his mother.
Danielle Wails, 21, admitted starting a blaze, which killed Alexander Gallon at a house in Newcastle
last year.
Wails initially clamed masked intruders who tied her up were responsible. She admitted infanticide
at Newcastle Crown Court and will be sentenced later.
An independent expert has now been commissioned to review the role played by council and health
officials.
The Local Safeguarding Children's Board (LSCB) is to conduct a "serious case review" into the
child's death.
It will examine the role played by agencies including the city council's social services department
and primary care trust.

FATHER has gone to the High Court to gain the right to sue local authorities after his daughter died
of a drugs overdose.

Melissa Strickson, 13, died in October 2001 after running away from home with three other girls and
taking refuge at the home of self-confessed white witch Sally Corkhill, of Sudell Road, Darwen.

Her father, Victor Strickson, who has started legal action against the police and Blackburn with
Darwen Council, claims the authorities failed to give them help in controlling Melissa, who ran away
from home 27 times.

Victor and his wife Sue, of of Tockholes Road, Darwen, say their requests for Melissa to be placed
in a children's home were denied.

Melissa had a massive overdose of coproximol tablets and was pronounced dead on arrival at Blackburn
Royal Infirmary.

Corkhill, 41, was sentenced to two years in jail after pleading guilty to four abduction charges and
two of administering a controlled drug.

She was cleared of manslaughter.

Her lover, Lee Harrison, 31, also of Sudell Road, received nine months on four charges of child
abduction.

Mr Strickson issued proceedings in Blackburn County Court in October 2002 alleging that his
daughter's death had been caused by serious failures on the part of the local police and social
services.

But in March last year his claim for damages was struck out by District Judge Law, sitting at
Preston County Court.

Overworked care staff fear another tragedy

SOCIAL services in Norfolk admitted yesterday they had failed in their duty to protect Lauren Creed,
the five-year-old child murdered by her mother's violent boyfriend.

Other agencies involved with the child in the three months before her death said they had made
mistakes, too, and police admitted a series of fatal errors.

Director of Norfolk social services, David Wright, said that the same mistakes could be made again
because of the immense number of domestic violence cases his department had to deal with.

"We failed in our duty, and for that I am deeply sorry," he said. "This could happen again because
our workload now is even worse."

Graham Sate, 25, a merchant seaman, was jailed for life yesterday for the murder of Lauren, with a
recommendation that he serve at least 25 years.

Sate, who battered the little girl to death, had been out of prison for less than a year, following
a sentence for attempted murder, when he killed Lauren.

The child's mother, Sharon Creed, 25, was jailed for five years after admitting two charges of
cruelty, one for failing for 90 minutes to call an ambulance to assist her dying daughter.

Following the sentencing at Norwich Crown Court, chief officers from Norfolk social services and the
area child protection committee, the civilan police and the RAF police all admitted mistakes had
been made after police and social workers failed to act on signs that Lauren was at risk.

Norfolk constabulary was criticised after two officers called to deal with a domestic incident at
the child's home at RAF Coltishall, Norfolk, three months before her death, failed to examine her
although they were told she was covered in bruises. They also failed to inform the probation service
of Sate's convictions for attempted murder and actual bodily harm. At the time Sate was still on
licence to Humberside probation service and could have been recalled to prison.

BABY P AGED 17 MONTHS DIED AUGUST 2007

Two men have been found guilty at the Old Bailey of causing or allowing the death of a 17-month old
baby in a case which highlighted one of the most severe child protection failures since the murder
of Victoria Climbié.

The abuse of the child, known as Baby P, was said to have taken place over eight months, during
which time the boy was on the child protection register of Haringey – the same local authority
that was found to have failed seriously in its duty of care to Victoria, who died eight years ago.
The 27-year-old mother of the boy pleaded guilty to causing or allowing his death at the beginning
of the trial in September.
One of the men was the 32-year-old boyfriend of the baby's mother. The man, who cannot be named for
legal reasons, was cleared of murder. The other man was Jason Owen, 37, who lodged in the same
house.
Both men have been remanded in custody to be sentenced on December 15.

At 11.35am on August 3 2007, an ambulance was called to the house. Its crew found Baby P already
stiff and blue in his cot. He was taken to North Middlesex hospital where he was pronounced dead at
12.20pm.
The Old Bailey heard he should have been protected by social workers, police and health
professionals, but his mother manipulated them with lies and even got away with smearing him with
chocolate to hide bruises.
In the 48 hours before the boy was found dead in his blood-spattered cot, it is claimed that a
doctor failed to spot his spine was broken.

ROMARIO AND DELAYNO MULLINS-SEWELL AGED 2 AND 3 MONTHS

A 21-year-old mother, being held on suspicion of murdering her baby and toddler has been sectioned
under the Mental Health Act.
The bodies of Romario Mullings-Sewell, two, and his brother Delayno, three months, were discovered
at a property in the Cheetham Hill area of Manchester just before 6pm on Wednesday.
Officers were alerted to the grim find at the house in Kilmington Drive after reports of "concern
for welfare" were made.

Couple jailed after baby's death
A couple have been jailed for three years for child cruelty following the death of their
six-month-old baby.
Stacey Taylor and Mark Harrison's son, Jack, died of pneumonia at Tameside General Hospital in
November 2006.
Examinations later found that he had suffered a number of fractures to his limbs and the Tameside
safeguarding children board launched an inquiry.
Small bruises caused up to 48 hours before he died were also found on his face and head which the
pathologist said was most likely caused by poking or prodding.
Speaking after the hearing, Det Con Kath McKenzie, of Greater Manchester Police, said Harrison and
Taylor had deliberately harmed their defenceless baby.
Jack would have clearly been very ill in the lead up to his death but Taylor and Harrison did not
take him to hospital or to a doctor, presumably because they were aware that the child's injuries
would be discovered," she said.
"Throughout this investigation they have been unable to explain how he got his serious injuries,
insisting they were either accidental or they did not realise he was injured.
"The court has found this is not the case. Maybe one day Jack's parents will take responsibility for
their son's tragic and preventable death."

A teenager who killed a baby when he spun her around and slammed her head against a wall had been
banned from seeing her, it has emerged.

The youth, now 17, was found guilty at Norwich Crown Court of murdering 11-month-old Ruby Spink.

He had been barred from visiting Ruby, who was placed on the child protection register days before
her death.

Ruby, who lived with her mother Laura Spink, 18, at a hostel in Great Yarmouth, died in September
2007.

Spink admitted cruelty and the judge said her care was "inadequate".

The boyfriend, not named because of his age, and Spink will be sentenced on 28 August.

The jury of eight women and four men took five hours of deliberation to reach their verdict.

Social services and police have been condemned for failing to stop a violent father who went on to
murder his 11-week-old baby boy.
Courtney Bryan was well known to the authorities in Nottingham where he was involved in four
families in which young children were seriously injured.
But a review of his case revealed there was a disastrous failure by social workers, police and
health staff to communicate with each other.
Police were accused of making slow and inadequate inquiries into his attacks on two babies while
social workers under-estimated the overall threat he posed. Had he been charged and punished for the
Nottingham attacks, the authorities would have been legally obliged to track where he was living and
the murder 35 miles away in Sheffield of his son Amaraye last year may have been prevented.
In fact, he was only charged for the earlier offences after he murdered Amaraye - a delay of seven
years.
The case is the latest to reveal agency failings and follows the death, after months of abuse, of
Baby P in Haringey, North London.
The serious case review, conducted by Sheffield and Nottingham's Safeguarding Children Boards, which
referred to Bryan as Adult A, said: 'There was no coordinated analysis of the risks posed by Adult A
and no strategic plans to address those risks.
The significant information that Adult A posed a danger to children existed but was not recognised
or utilised. In particular, the investigations of several of the injuries were not completed quickly
enough to charge him at the time.

Earlier this month 32-year-old Duncan Mills was found guilty of murdering his month-old son Luigi on
May 26 last year as well as battering the baby's mother Samantha Askew the same day.
Following little Luigi's death a statutory report on how the issue was dealt with - a Serious Case
Review - has been drafted by Suffolk's Safeguarding Children Board and a meeting to discuss the
findings was due to take place today.
As revealed previously in The Evening Star, Suffolk County Council would not release the findings of
the public document until after the board had met to discuss it.
Back in 2002 killer Duncan Mills received a two-year prison sentence at Ipswich Crown Court after
admitting viciously assaulting his then girlfriend on Valentine's Day.
During the assault he tried to force a mobile phone into her mouth and punched her in the face.
One of the questions hanging over Suffolk County Council social workers before the publication of
today's report was whether social services were aware of Mills' violent past and if this was taken
into account when assessing baby Luigi's safety.
During this month's murder trial it emerged Suffolk County Council social workers had been in
contact with Miss Askew over concerns she was a victim of domestic abuse at the hand of former
rapper Mills, who lived in London Road.
However Luigi was deemed to be “well cared” for and no action was taken.

A BABY girl who was killed by her father was removed from the at-risk register despite a social
worker’s pleas, a damning report revealed.
Inspectors criticised a catalogue of failings in the treatment of little Alisha Allen. They found:
Senior social workers failed to attend a vital conference into her protection
A doctor failed to report a bruise on the baby’s forehead
The abusive past of Alisha’s father was ignored
Sunderland Royal Hospital discharged her after treatment for weight loss despite concerns about her
welfare.

Sixteen-month-old Amy Howson died in December 2007. Her father, James Howson, 25, was found guilty
of her murder and sentenced to a minimum of 22 years in prison.
Her mother, Tina Hunt, was given a 12-month suspended sentence after admitting child cruelty.
Amy died after her spine was snapped in two, but prosecutors said she had suffered for months before
that.
When she was examined in hospital on the day she died, she was malnourished and dehydrated.

Little Sanam Navsarka was assaulted over a month-long period and died with fractures to all her
limbs.

Prosecutors say her mother Zahbeena Navsarka and her mother's partner Subhan Anwar, both 21, from
Huddersfield, West Yorkshire, did nothing to save her.

Both are charged with murdering the two-year-old.

They deny the charges and Anwar also denies a charge of causing or allowing the death of a child.

Navsarka has admitted an alternative charge of failing to protect Sanam.

A FATHER who stabbed his two young sons to death before trying to set their bodies alight was today
jailed for at least 21 years.

Ashok Kalyanjee, 46, admitted murdering Paul Ross, six, and his brother Jay, two, at an earlier
hearing and was sentenced at the High Court in Paisley today.

There were shouts of "yes" from family members sitting in the court as Lord Brailsford jailed
Kalyanjee for life and told him he would have to serve a minimum period of 21 years behind bars.

Kalyanjee lured the reluctant boys into spending the day with him and drove them to a secluded spot
and cut their throats last May.

He then poured petrol over his sons and himself and tried to set the silver Mercedes on fire.

Her killer, Her mums brother-in-law admitted killing this much loved baby and her family were
horrified to learn the killer had swung Maddison around by her ankles and used her as a football in
the living room. He puts his monstrous actions down to being drunk. He recieved life in prison with
a minimum of 25 years to be served.

Leanne was beaten & murdered by her mothers new boyfriend (TERRY LANGRIDGE) to which Leanne's mother
knew about & let happen! She would hide the marks on this little girls body. Till the day came her

Daniel Muir aged 23 months

ORE than 30 injuries were found on the body of a toddler whose mother is on trial accused of killing
him, a court heard today.
One juror cried as details were read out of bruises, abrasions and scratches found on 23-month-old
Brandon Muir by medical staff.

The High Court in Glasgow also heard that the toddler had a ruptured intestine and fractured ribs.
Brandon's mother, Heather Boyd, 23, is accused of the culpable homicide of her son by allegedly
failing to get him medical help - a charge she denies.
Co-accused Robert Cunningham, also 23, who was her partner at the time of the toddler's death,
denies murdering Brandon by assaulting him in a flat in Balunie Crescent, Dundee, in March last
year.
On day 11 of the trial, consultant paediatric pathologist Dr Alan Howatson told the court Brandon's
ruptured intestine may have been caused by "a single trauma".
The jury was then shown photographs of Brandon's injuries, which were described by Dr Howatson, who
read out 36 paragraphs from his notes - some of which described more than one injury.
The court heard that Brandon had bruises on his head and abrasions on the inside of his eye.
He also had bruised arms and circular groups of bruises on his abdomen.

Rhys Biggs ~ aged 2 months

Evil Paul Husband, the boyfriend of Rhys’s crack addict mum Claire Biggs, was found guilty in
Scotland in 1993. But his past was NEVER checked by social workers.
And even if it had been, Scottish cops had not put the conviction on the Police National Computer.
It was only AFTER Rhys died with a string of horrific injuries that the truth came out.
A Scotland Yard source said an initial check on Husband on the national computer drew a blank.
An officer then ran a second check covering Scotland.
The source said: “When he did that it sprung up there was a rape against a minor.
“That should have been on the computer from the start.”
Rhys died in May 2006. During his short life he came under the care of two London boroughs —
Camden and Newham.
After the breakdown of Biggs’s relationship with Rhys’s biological father, she became involved
with Husband.

Rilee Saunders~ aged 5 weeks

A father from Cornwall has gone on trial accused of murdering his five-week-old son.
Exeter Crown Court has been told Rilee Saunders died from serious head injuries at his home in
Quarry Street, Torpoint, last March.
The prosecution said he suffered a fractured skull and brain damage by being struck or being struck
against something.
His father, Brian Saunders, 31, has pleaded not guilty to murder.

Leeya Akinrele~ aged 7 weeks

A SEVEN-week-old baby died after suffering a fractured skull, 20 broken ribs — and human bites to
her face and fingers which turned septic, a court heard yesterday.
Leeya Akinrele had “catastrophic brain damage”. Her fingers had been bitten so hard the bones
fractured.
Prosecutor Yvonne Coen said: “The injuries were inflicted by one or both of her parents.”
Jobless Kelly Inman and Olusola Akinrele, 33, from Whittlesey, Cambs, denied murder at Cambridge
Crown Court.
Inman, 21, admitted allowing the death of a child. Trial continues.
